You may qualify if you or your loved one
- Has had a recent decline in the ability to manage daily activities
- Has a history of multiple hospital admissions or an unstable medical history
- Needs help such as a nursing home aide, social services or rehabilitation support
- Is homebound
- Is a medically complex child
- Is an adult child caring for a parent and needs help managing their needs
Programs and approvals
Some clients receive care through state and local programs. You may qualify if you are:
- Approved through South Carolina Community Long Term Care (CLTC)
- Approved by a Disabilities and Special Needs Board (DSNB)
What may be required
Depending on the program, qualifying may involve:
- A physician’s order, plan of care and certifications
- A face-to-face encounter
- Confirmation that the person is homebound
We will explain exactly what your program requires during your free in-home consultation. Families who pay privately can also arrange care directly. See who we serve and how families pay for care.
